Case Management
Case managers can help seniors and adults with physical disabilities, who need assistance managing activities of daily living, access the services and resources they need. Services include:
  • A visit to your home to assess your needs and develop a care plan
  • Arrangement for you to receive supportive services such as home-delivered meals, transportation and/or home care
  • Your case manager will help you access the services you need from Medicaid
  • Referral for other community-based services
  • Evaluation of your benefit / entitlement needs
  • Advocacy with landlords, utility companies, benefit programs
  • Counseling for long-term care issues
  • Regular contact with you to ensure that the services you are receiving continue to meet your needs
Case Management services are available through Medicaid, Oregon Project Independence, and Aging and Disability Resource Connection (ADRC) / Senior Connections. Depending on your eligibility, Case Management services are provided to you at no cost. You can also find out about private pay case management through the ADRC.

Options Counseling
Options Counseling is a free service provided though ADRC to all seniors and people with disabilities (18 and over) to help them plan for their long-term care needs. These services are provided by telephone or in person, which includes home visits. If you would like to speak with an Options Counselor, please call 541-682-3353.
